A comprehensive chemical use assessment is available in each of our outpatient locations.  An assessment summary, which includes a brief description of the individual’s bio-psycho-social history, chemical use history, collateral information, diagnosis and recommendations, will be provided to the referral source and the individual upon request.  Recommendations and resources available will be discussed during this process. There is a cost for this assessment if a report is generated.
